REGISTRATION
	Legal Member Agreement To Participate In The Programs of Guelph Minor Hockey Association I/My participating child hereby agree to abide by and support the current Ontario Minor Hockey Association (OMHA) Code of Conduct, and rules, regulations and decisions of the OMHA, Ontario Hockey Federation (OHF), Canadian Hockey Association (CHA), and the Guelph Minor Hockey Association. The OMHA Code for Conduct identifies the standard of behavior that is expected of all OMHA members and participants. The Guelph Minor Hockey Association is a member of the OMHA which for the purpose of this policy shall include all players, guardians, parents, coaches, officials, volunteers, directors, officers, committee members, conveners, team managers, trainers, administrators and employees involved in OMHA and GMHA activities and events. GMHA is committed to providing an environment in which all individuals are treated with respect. Members and participants of GMHA shall conduct themselves at all times in a manner consistent with the values of OMHA and GMHA, which include fairness, integrity and mutual respect. During the course of all OMHA and GMHA activities and events, members shall avoid behavior, which brings OMHA or GMHA or the sport of hockey into disrepute, including but not omitted to abusive use of alcohol, use of non-medical drugs and use of alcohol by minors. GMHA members and participants shall at all times adhere to OHMA's operational policies and procedures, to rules and regulations governing OHA events and activities, to rules and regulations governing any competitions in which the member participates on behalf of OMHA, and to the GMHA Bylaws, Rules of Operations, policies, and procedures that govern GMHA . I as a member and participant of GMHA shall not engage in any activity or behaviors which interferes with a competition or with any player or team's preparation for a competition, or which endangers the safety of others. I as a member of GMHA shall refrain from comments or behaviors, which are disrespectful, offensive, abusive, racist, or sexist. In particular, behavior, which constitutes harassment or abuse, will not be tolerated, and will be dealt with under OMHA's Harassment policy and GMHA's Bylaws and GMHA's Rules of Operations. I as a member and participant acknowledge that from time to time I / my child will be traveling from place to place, sleeping away from home, and eating away from home. I / my child may reserve the right to take action against any wrong doer but, subject to that, I release the OMHA and the Guelph Minor Hockey Association and including all members, coaches, managers and association executives from any responsibility. I hereby understand and appreciate that participation as a hockey player carries risks to me or my participating child of serious injury, including permanent disability, paralysis or death. I/My participating child voluntarily and knowingly acknowledge, accept and assume these risks. Failure to comply with this Member Agreement may result in disciplinary action in accordance with the Discipline Policy of OMHA and the Bylaw and Rules of Operations of GMHA. Such action may result in the member losing the privileges, which come with membership in GMHA and OMHA, including the opportunity to participate in GMHA and OMHA activities and events, both present and future. I the participant/parent have become subject to the rules, regulations, and decisions of the OMHA, OHF, CHA, and the Guelph Minor Hockey Association and it's executive which may restrict in some areas such as movement from team to team, conduct, etc and agree to abide by such rules, regulations and decisions of the OMHA, OHF, CHA, and the Guelph Minor Hockey Association and it's executive.
